Reports

After a detailed and thorough examination of the documentation, and generally after a pre-arranged teleconference with the solicitor, counsel or both, a detailed report is written based upon the Assumptions and the evidence of the records.

The test of the standard of care that is applied is that which would be acceptable to a wide body of peer general practitioners practicing in Australia at the time the services were provided.

Reports are presented in the following format:

The Declaration appropriate to the jurisdiction of matter is stated.
The Documents that have been received are listed.
The Assumptions are re-stated.
Any principles of medicine required to understand the rationale of the report are explained under Relevant Medical Science in simple but accurate terms.
The Chronology of Events is tabulated.
A detailed, chronologically ordered Analysis and Discussion follows, bringing together the Assumptions, Chronology and relevant medical science from Relevant Medical Science.
Breaches are identified and highlighted throughout the Analysis and Discussion.
An Opinion is given for each of the questions specifically asked, supported by the rationale of the Analysis and Discussion.
References to support the Analysis and Discussion, as required, are listed in standard format within a Bibliography at the end of the report.
A current Curriculum Vitae is attached to each report.

Supplementary reports are written as requested.

Report completion dates are negotiated with the client and are prioritised according to urgency to fit in with clients' needs.
